In a statement released on Monday, Fifa confirmed that Leslie Sedibe had been found guilty of infringing several codes of conduct, and banned him from taking part in any football-associated activities for five years.
Sedibe was also handed a 20 000 Swiss Franc (R311 132) fine.
The infringements include contravening Article 13, which is the General Rules of Conduct, Article 15, which relates to loyalty, and Article 18, which refers to the duty of disclosure, co-operation and reporting of the Fifa Code of Ethics.

Former SA officials Steve Goddard and Adeel Carelse have also been found guilty of the same offences and each received a two-year ban.
The bans follow an investigation that was initiated by Fifa in November 2014 relating to, among others, Bafana Banana’s 5-0 thrashing of Guatemala in May 2010, ahead of the World Cup.
“The adjudicatory chamber of the independent Ethics Committee chaired by Hans-Joachim Eckert has banned several officials from all football-related activities (administrative, sports or any other) at both national and international level,” the statement said.
“Three cases were decided concerning international friendly matches played in South Africa in 2010 . . . All bans come into force immediately.”
Last year, Fifa also banned Lindile ‘Ace’ Kika, former Safa head of national teams, for six years.
